SSM's wheelchair-accessible Adult Day Services facility in Fond du Lac offers mental health services with group therapy and psychiatric services. This location provides day treatment, also known as partial hospitalization (PHP).
They are open weekdays from 7:30 a.m. to 5:30 p.m.
Clients can access care notes during and after treatment by logging into MyChart, their client portal.
